The form I used to use for this is broken and cannot be fixed. Long story. For now, just send me an email with the following info:

• First Name

• Last Name

• Date of Birth

• Last 4 of your Social Security #

• Desired Password (must be at least 6 characters)

• A valid email address (NOT City of Irving email)

The personal information is for identification verification only. I will check it against association records and then enter your password. As soon as it gets entered I will send a confirmation email to you. It may take a day or two for me to get it entered in the system.
